Please include, if available:

your name and preferred contact method;
order number;
product name and lot or batch number from the container;
when and how the product was used;
what happened and when;
whether medical attention was sought;
photographs of the product and label.

We may ask for permission to contact a healthcare professional and may need to share relevant information with the responsible manufacturer, fulfilment provider, insurer, adviser or regulator. Product-safety information is not used for targeted advertising.
